Potholes, Other Street Repair

There is a pothole on the east side of High Street between Southdowns and Woodstock. The pothole is near the curb and near one of the utility poles. It is not the largest pothole I have recently seen but it is large enough to severely rattle a passing vehicle. It is also hard to avoid if traffic is coming from the north as one must cross over the center line to avoid the pothole.
Jennifer Blythe sent the following DM on FB this past Sunday, September 30: accessibility issue At Lotus, I was using my wheelchair. I was THROWN from my chair TWICE while trying to negotiate supposedly accessible curbs, and had to stand multiple times to get over cracks in sidewalks. Luckily, I am not completely wheelchair bound, YET... but I think this needs immediate attention! This curb is on 6th street in front of the genealogy center.
There has been a giant pot hole for months at the south end of S. Sims Drive, just as you turn off of Fullerton Pike onto S. Sims.
There is a large bump in the concrete outside of my house, one in front of my driveway and one in front of the mail box that is causing damage to my car and in a bad spot for the mailman. The address is 3041 N. Cantlin Drive, Bloomington, IN 47404. Thank you!
Multiple pavement issues in culdesac of W Mill Run. See picture. Also, when you turn onto N Union Valley Rd from 45/46, the right lane has a bunch of potholes in one spot.
At the intersection of Twin Oaks Valley and Twin Oaks RDG in the Adams Hill Subdivision the asphalt has broken up in a large area and is forming a hole. The asphalt needs to be removed and replaced.
Several potholes at the northwest corner of the circle drive around Showalter Fountain, in front of the Fine Arts building on the IUB campus.
Across the street from our house, there is street parking, but there is a lot of broken glass. Not only is it a safety hazard for people walking/crossing the street, but there is definitely enough glass to cause popped tires. If it can get swept/cleaned up soon, that would be amazing.
The 3 Alleys between S Madison's dead end (North of Grimes), connecting to Rogers St need potholes filled.
