Potholes, Other Street Repair

Cory Lane between 2nd and 3rd. Multiple potholes 6 or more inches in depth.
There’s a smallish but deep pothole in front of the building at 314 W 4th St, garage entrance.
Small ones at westbound spea crosswalk and north n Smith right before large apartment complex near 10th
1600 block of E First Street: pothole at one of the speedbumps
4201 Sheffield but on the Plymouth Road side of property (corner lot). A few feet to the north of the driveway, the street has begun to sink and heave to the point where the curb has raised at least 4 inches and broken into pieces.
This intersection has long been dangerous, but the improvements lately have been GREAT and much improved safety. However, when the crossing sign was installed on this corner, the road was never filled back in. We're only into December, and this pothole is like a crater when riding a bike. Because it is right in-front of the crossing sign, one must go into the pothole in order to activate the button, and then it takes much longer to get out of the pothole and enter the intersection. It's taken me long enough at least two times that vehicles which had slowed to allow me to pass actually started progressing into the intersection again because I couldn't navigate out of the pothole quickly enough. This totally null-and-voids the enter purpose of the signal enough that I've temporarily stopped using it in daylight hours. Even just a temporary fill or cover would be helpful until it can be repaved/sealed. (at the northeast corner of intersection of Rogers and W 4th Street, specifically on Rogers.)
Turning onto Hillside from Walnut: multiple potholes can be found, continuing all the way to the intersection with high Street.
There are a number of potholes on Penn Court near the driveway of 4117 E Cambridge. The holes go all the way up Penn court to our neighbor’s driveway
