Growing Pothole in westbound lane on tapp going down the hill towards the roundabout

In the westbound lane of traffic on Tapp Road, just west of the Kingdom Hall, there is a big pothole on the north side of the lane, getting bigger. This was reported weeks ago and never fixed. It needs repaired. Thanks.

Sunken grate is like a huge pothole. It's right against the curb on the east side of the road (north bound lane). The storm grate here is 3-4 inches below the surface of the street. Really nasty bump if u hit it, even going very slow. My thought is that you guys could just lay an identical grate on top of it, the gap is about the width of the grate, rather than tearing up the road. Thanks, you guys are great!

Large pothole on east side of South Park Ave between Grimes and Wilson. To avoid it, northbound drivers must move to the far left side of the road, making Park a 1-lane road at the point of the pothole (around 1300 S Park)

Hello. When the team was here to repair the sidewalk two weeks ago, they left behind a washout bag full of concrete mix. Due to the rain, that mix is now filled with water and it is too heavy for me to lift. Please send someone to collect and dispose of the bag as the repair has been completed for over two weeks.
