Residential street light out

I have contacted the city twice about a street light that is out frequently, and while I have gotten a quick response and a promise to contact Duke, nothing has been done to fix the problem. The light has been out on the following dates (and probably more, but we were out of town a few times during this period): October 19, 21, 22, 29; November 2, 5, 9, 11, 12, 13, 14. It is very dark on our street when this light is out, and I would appreciate some more active follow-through with Duke to get this fixed as soon as possible.

Storm Tree Debris cleanup. Debris located on east (Ridgeview Dr. ) and north (Sunny Slopes Dr.) sides of the property.

Pine tree is down on the church property just off the driveway to the right side. Tree trunk and limbs are completely broken down into manageable pieces. Assistance to remove it would be greatly appreciated. Please let me know if you have any questions or have any special requests. Note that the picture was taken before the tree was completely broken down.
