During the most recent snowfall (between 3/16 and 3/25 can't remember exact date around 6am). I hit a pot hole on Fullerton Pike between Rockport and HWY 37. Caused damage to the right bottom of the bumper. I have seen the crews out there patching it up at least twice over the last week. Talked to insurance company about repair and was told to report it to Bloomington DPW. What is the process to submit a claim for damages.

Street remains over half blocked by tree on power line. Duke Energy knew nothing about it. Why hasn't the city reported this to Duke? This is a heavy traffic road that's down to a single lane with no warning signs.

Someone has spray painted the word war on three stop signs in Winslow Farm - one at the corner of Laurelwood Drive and Wylie Farm Road, Bent Tree Drive and Wylie Farm Road and Bent Tree Drive and Hickory Stick.

There are two very large and deep muddy puddles in the large dog park area at Switchyard. One is located by the large tree in the center of the park, and the other is located by the sitting area under the green awning. My dog recently got into the muddy water, ate and drank it, and became severely sick for several days. After speaking with several dog owners at the park, this has happened with their dogs too, so it is definitely a health hazard. It is very difficult to keep the dogs away from these areas. This does not seem to be a problem just after rainfall, but most of the time. I ask that they be filled with gravel or fenced off to allow for regrowth and to keep dogs away in the meantime. Thank you!

I submitted an erosion/drainage issue behind my house (2201 E. Autumn Dr. Bloomington IN 47401). Someone from City called and left a message for me saying that it is on private property therefore City will not take care of it. I showed it to several other folks and in their opinion, it is City's responsibility since the rainwater pipe is the one causing the damage. I am not sure if your site visitors went to the location to verify this. The rain water from the entire neighborhood comes through this pipe and ends in the creek. I am attaching a picture with this form so that you can take a look at it.If the site visitor wants to visit again, this is happening in between (the back of ) houses 2201 and 2200.

There are several street lights in the vicinity of the Baptist Church (maybe the name has changed) which is near the intersection of Woodscrest and 3rd Street that are not working. Could 3rd street in that area be driven at night to take a total tally of the lights that are out between High Street and College Mall Road? There are two deep potholes on the downhill entrance to Cascades Park. Lots of bicycles go down this hill at a high rate of speed. This is a very bad place for two deep potholes. Since they could cause a serious accident they should be taken care of as soon as possible.
