Sidewalk & Curb Complaints

I submitted a request for repair of this sidewalk in front of the house with no response. So then I submitted a 50-50 request to get this repaired, also with no response. I have already seen a child fall and skin up his hands and knees due to this sidewalk which has been like this for so long. It got worse when a city work truck came over the curb to collect brush and scooped so deep that the big root was pulled up and worsened the crack. Several surrounding slabs are cracked, but this one is the worst. Who do I need to contact to get this hazard repaired? Nothing so far seems to have worked. Thank you for any help or information you can give me.

Potholes, Other Street Repair

There is a manhole cover here that keeps getting left open. Last fall my husband fell in it and injured his leg. Can this be made safer? Mopeds, bikes, and pedestrians use this area with some frequency. The manhole cover can be hard to find, it is not far from the retaining wall near the Stonebelt house on the south side of the street.
