how do I get a replacement for my lost 1099R
7th and woodlawn, west of intersection, on 7th at union building, where busses pick up. Multiple holes next to curb. 10th st east bound across from crosstown plaza. 1 big hole at curb. Eigenman hall east bound at bus pull off. 1 big hole.
The alley way running between 219 E 7th Street Pavilion Heights & the neighboring property, The Manor, has developed a large pot hole coming out onto 7th Street and deterioration is occurring as well. Starting to cause potential safety and damage issue for motorists of both properties.
Follow up on #167661: I got this email "Sean Starowitz contacted Sean Starowitz I received your Ureport. Yes, we are waiting on the weather to warm up to finish the Traffic Boxes. This tends to be a learning curve for new artists and some were not able to get started (or did) before the weather turned. We have a few finished - 2nd and College and Kirkwood and Adams. The other four are in the works. " The box at 2nd and College is the one I asked about. It is NOT finished. One wide sign is painted; there is another wide side side and the two narrow sides left to prime and paint. All previous boxes have all 4 sides (and maybe the top) painted. This new box is bigger, but that's no reason to scale down. Thanks.
On 7th Street, west of Clark: pothole
On Jordan, starting three blocks south of the intersection with E Third Street, there are several places in the road that are subsiding.
It is so sad that the weather keeps un-doing all the wonderful work that the pothole crews are performing daily. For instance: On First, parallel to the Kroger loading dock, between Morton and College, there is a pothole again.
Another repeat repair request, caused by the weather: On E First in the 1300 block, near the intersection with Jordan, near the Little Library and the bench: several potholes.

This is a follow-up. The initial report through this portal was on 2/11/19. It is case #167396. This situation is about to get worse as the weather improves and the vegetation begins to grow. This is very dangerous and needs attention. This is the text from my original report: Our property located at 805-807 N Smith Rd has a blind driveway. As you can see in the picture, you are NOT able to see oncoming vehicles as you try to exit the driveway. This needs to be cleared and/or needs a sign near the convenience store about a blind drive. It is very dangerous.
