This was "patched" after I last reported it and it is back, as a threat to bicyclists since it is in the east-bound lane.

Multiple potholes in this stretch of roadway, present for the past month and likely to get worse in the ensuing snow/ice/cold snap

Just south of the Switchyard Park entrance on Rogers, in the southbound lane, there are various holes in the road. They seem like they were cut into the road, not necessarily naturally occurring potholes. But, these are an issue that needs to be addressed.
On the bypass heading west, there is a significant pothole on the left lane that needs filled.
It won't let me choose the location on the map. Headed northbound on I-69 in the right lane, just south of mile marker 113.2 is a pothole that has the potential to damage a vehicle's tires. It rocked the car. We called 911 on 1/31 to report it but just hit it again this evening. Please forward to INDOT to request as a priority repair or temporarily block the lane until a repair can be done. Thank you.
While redoing the Templeton elementary playground over the past year and a half many heavy construction vehicles used the two blocks of South Park Ave as their main access point for construction. During that time the road conditions on those two blocks of South Park ave (from Grimes to Hillside) have worsened becoming hazardous, with many potholes that need to be avoided like obstacles to dodge left and right. There are no sidewalks on this street so many kids walk to Templeton in the street, with the potholes creating further distraction for drivers. The construction crew hired was very respectful of the neighborhood, but the additional wear from the large project needs to be prioritized and addressed. Thank you for your time.
There is a hole (currently marked by an empty coke box) in the alley way between Bunger & Robertson and the 4th Street Parking Garage.
