I appreciate the additional screen shot for a picture is worth a thousand words. (so they say!) I am including an updated picture to illustrate the problem. It has happened before so I will be sure to let utilities know as well as the author of the software. Hopefully in the future update it will be changed so that there is not this confusion. Here is the issue: For Name information, the labels for the two fields (First, Last) are to the left of the first field. You, and everyone else, seems to enter data correctly here. But for some reason, on the city state and zip line, the three labels (City, State, Zip) are again to the left of the first field. But many people put all three values into the City field, leaving the State field as the default, and the Zip filed empty. So the solution is to just put the City in the first field, use the pulldown to select the State, and enter the Zip into the third field of that line. Thanks for taking the time to submit the issue.
