Other: On two recent occasions, emergency response teams were sent to 611 N. Rogers, apartment 104. These were in response to mistakes by a Life Alert notification, and unnecessary dispatch. The responders damaged the door to gain entry only to find there was no emergency. They informed us t hat there is a form to submit to file an insurance claim to pay for the damages. WE gave the form to the apartment manager. The manager did not file the form and claims that she never saw it. We need another copy of the form and cannot find it on the city's website. Please email me the form or a link to the form so I can print it and once again give it to the apartment manager. Thank you!
