Phone Call: Lee Starnes - Maintenance Supervisor for Fountain Blue Nursing Home called and said they have experienced a problem with bath facilities black staining and ice machine filters clogging and needing changed more frequent than in past.

Samples taken on 12/14/98. All results normal. SG called Mr. Starnes and advised him all sample results were normal. Advised Mr. Starnes to wait a while and see if problem improves over time. 01/22/99 - SG called and talked with Mr. Starnes. "Any improvement" "No". Sg asked Mr Starnes to call back next time he changes filters. No activity with this file last 4 months- case closed.
