City Construction Projects: Request
Phone Call: Mr. Turpin called and reported that when the Utilities Dept. installed new line on Rockport Road they dug up entry to his driveway. At that time the driveway had a culvert. When redoing his driveway, no culvert was installed. Water backs up in his driveway now and he has a "muddy mess." He would like a culvert installed since he is afraid his car will be damaged.

I referred Mr. Turpin to Risk Mgmt. Dept. to report damage. I gave copy of citizen action order to Mike Bengtson, Asst. Dir./Engineering. I also gave Mr. Turpin my name and telephone # to call if he has any problems reporting damage. Mr. Turpin called CBU on 9/10 and stated that Risk Mgmt. was to call him back but he has not heard from that Dept. I contacted Jennifer Lloyd, and she will call Mr. Turpin. - nlw 9/11/98 - Risk Mgmt. is handling this matter. A claim form has been sent to Mr. Turpin. Jennifer Lloyd will call Mike Bengtson as soon as she hears from Mr. Turpin. - nlw **** NOTE: As of 10/27/98 Mr. Turpin has not answered.
