Phone Call: 4/25/97 - 8:30 am. Telephone call from Mrs Malone. Water at Surgery Center is "murky". Thinks it may be associated with work at 37 & Tapp Rd by PSI.

1. 04/25/97 - SG directed T&D to flush vicinity hydrants 2. 04/25/97- SG called lab to send Doug to collect sample 3. 04/25/97 - SG visit to customer - Checked chlorine: 0/1.0 mg/L SG called mariam malone - advised her test of water showed it was slighly above recommended level for iron. (0.35 mg/L.). Mariam said their water was clear & back to normal now.
