Report of a clogged driveway culvert and resulting errosion. I have verified the problem and determined that work performed by Reynolds,Inc. for CBU contributed to the problem.

Did site inspection and contacted contractor. Contractor sent someone to remedy the problem. Received further complaint about a week later. Met Carol Jack at site on 10/25/96. Believe leaves were clogging up culvert. Carol Jack was to contact tenant. B. Reinhold contacted contractor and gave him complainant's phone # to work out problems. Complainant did not call back.
