Along the sidewalk and next to a utility pole is a plate on the ground installed by the fiber optic company. At the SE corner of the plate there is a gap between it and the dirt, large enough for a small shoe or animal to step into and be injured.

Detour around closed section of B-Line Trail is not accessible. Following the detour leads pedestrians across a broken tactile curb ramp, across road covered in gravel. The makeshift ramp on the Hopewell side of the street was not able to be climbed by my wheelchair. I couldn't go West around it down 2nd Street on the north sidewalk because it's blocked by stones in the sidewalk. Couldn't go West around it on the south sidewalk because the sidewalk is blocked. Couldn't go East on the north sidewalk because the Seminary Park building has a big drop-off on the sidewalk. I had to ultimately use the Wendy's/Kroger parking lot to get where I needed to go. I'm quite frustrated that detour accessibility is not being planned ahead of time, before a detour is put into place
We have a gigabit line install over the city sidewalk, easement. Installed by axis solutions Nov 25th. This is a heavily used sidewalk near mailboxes and we have two blind residents that use this pathway. There is a hello flag but no safety ramp, cones or other alerts that might deter a blind person. It's not a well lit area and is a tripping hazard. Please call at your earliest convenience, 812.650.8740 to offer solutions.
The contractor doing underground work (fiber?) left a bunch of loose soil along the west side of Walnut St Pike. They came back and seeded and strawed some mother areas but not this.
This huge break in the road was not properly marked - the cone was well over a foot from the edge - and I hadn't realized how far in it went until it was too late to avoid it. Two of my tires popped, and my car had to be towed. I'd like the city to reimburse me for replacing them, but more importantly, I'd like for them to make sure these holes are properly marked.
Gigabtye Contracted companies placed these while laying lines but have not returned to address their lack of filling in the side. This one just a great example but many if not all are this bad. Easy to fall into or wreck your bike if you don’t see the ruts on all sides. Especially for those of us who walk/ride at night. Where’s the oversight if any? Please fix these.

Duke Energy has refused to remove a large orange cable that is down on the length of my property (3/4 of an acre). They say someone else needs to take care of it, like maybe a cable company. I am concerned that it is dangerous to kids in my neighborhood and I cannot do yard work around it. Moreover, it is in the utility easement. They said to pick it up and see if it is attached to the utility line and I am unwilling to do that because it seems risky. In any case, no utility should be leaving their waste on my property and they should take responsibility for removing it safely.
Cylindrical hole about 4" radius drilled into street by fiber-optic workers and left unfilled. Dangerous area because there are no sidewalks along at that location, so people are forced to walk on the street. Hole is closest to SW corner of intersection. Ankle-breaker!
