Dear Rhea, my address is 4426 E. Sheffield Drive 47408. I am also appending a picture. I would normally not bother, but the can is still half full. I actually had entered the address in the window of the original request, but somehow it disappears (I had to enter it twice). I replied to your e-mail request, but noticed this morning that your request came from a "no-reply" sender. I tried to open the u-report case, but couldn't. So I am simply filing a new request.
