My name is Steve Egyhazi [email protected] and I live at 3922 S Woods Edge Bend in Bloomington 47401, cellphone 812-340-0759. I am hopeful this form is the correct place to submit this concern: On or before 2/21/2020 I noticed that my front-yard sewer plate thing is all ripped up and askew like I've never seen it before and it just don't seem right.
