Water running out of ground for a week.

There appears to be a broken water main because water has been rising to the surface on my property the past 5 days. My front north sidewalk (photo attached), east sidewalk, and south concrete garage floor have been soaked for the past 5 days. My basement is dry and no pipes are leaking in my house. Since the street was repaved our garage has flooded 3 times in the past year due to runoff coming d9wn our driveway and over our yard. We tried to dig a relief ditch but this is more difficult and the flooding is due to the fill in of the original ditch in the easement and the fact that the culvert under the driveway is now clogged with roots and dirt. I need the ditch dug properly. Also, your department fixed a brok3n main last winter and did not fill in the hole left by the repair.

Drainage ditch near the street in front of my residence has developed deep holes over the past several decades that I have attempted to fill continuously wash out. Could there possibly be in break in the water main? Thank you for looking into this matter.
