During the evening of April 12th around 10:30 pm, I was driving home in the south bound lane when we hit a pothole right after house number 8495 on North Old State Road 37. Both of my rims on the passenger side were bent because of the pothole.

There's a 2'x2' square pothole forming (maybe over a former sewer).
3 potholes in a hole, almost all the way across Moody Drive, once you turn onto Moody from Henderson
pothole west bound lane 600 block E.Grimes (about 100 feet east of Grimes-Henderson intersection)
Pot hole right in front of a non A.D.A compliant wheel chair ramp. hole is right in front of I.U.Hospital parking garage entrance..... Electric wheelchair gets stuck in hole{in the street} while trying to get up the ramp. Please fix this problem as soon as possible, thanks Brian Wilson
Just east of the intersection of E 2nd and S Woodscrest Drive in WB lane. Officer advised it was not too bad if drivers were going the speed limit but still deemed being checked. Thank you!

The entrance of the alley off of Howe heading South has a few potholes and loose gravel. It is the alley between 521 and 601 W Howe.
