On Rogers along the Switchyard property the road is getting pretty beat up in the northbound lane. In one spot, the entire side of the road bed has fallen away. This is dangerous because it's where cyclists ride towards the outside of the lane. North of that damage, there is a grouping of pot holes forming that are getting pretty bad. These all need fixed ASAP.
There's a water meter or something in the road that has a pot hole around it. It's in the east bound lane of Country Club Drive just west of the B-Line Trail crossing. You'll see it.
There is a section of pavement in the bike path / walking path / running path on Southdowns just east of Woodlawn Avenue. There's a big gap on the pavement. It is now big enough a bike tire could get caught in it or a person walking might find the uneven ground tough to navigate.
On Covenanter: near the intersection with Pickwick: pothole at the speedbump. Also, near 2312 Covenanter, another one near the speedbump.
North to South Alley between 5th and 6th street , College Ave. and Morton St.

Long gap-like pothole in the middle of the street. Dangerous for bicycle traffic.

There are a number of potholes that need to be filled in this spot. It doesn't just need a few to be filled, but needs to be completely paved over for this stretch of potholes. Because there are so many potholes, when the spot fills with rain it's impossible to spot hazards and creates hydroplaning.
