Please accept our sincere regrets on your recent loss. We would like to help your family select a new pet and will waive the adoption fee for one animal.

Please submit the attached form and it will be emailed to the Animal Shelter, or print it out and mail it to City of Bloomington Animal Shelter at 3410 S. Walnut Street, Bloomington, IN., 47401.
